1. DS82 form – Completed and printed
2. Letter of Authorization – Manually filled and signed
3. Your existing passport book and/or card
4. Statement for a Second Passport – Manually filled and signed
5. Two new passport photos (no glasses)
6. $190 Check to US Dept of State (or $220 for Book and Card)
7. Copy of Proof of Departure
